Join us at the Greenfield Public Library on Tuesday, April 21 at 12:00 pm for “Understanding MOLST Forms” with Dr. Khush Dhaliwal.

Dr. Khush Dhaliwal, a local family medicine physician resident at Baystate Franklin, will be leading a session on Medical Orders for Life-Sustaining Treatment (MOLST) forms — what they are, who they’re for, and how to think through the decisions they involve.

MOLST is a medical order completed with a healthcare professional that outlines a person’s preferences for treatments such as CPR, intubation, mechanical ventilation, and other life-sustaining interventions. Unlike a general advance directive, a MOLST form is intended for individuals with serious illness or advanced frailty and travels with the patient across healthcare settings.

During this session, we will review what each section of the form means in practical terms, clarify common misconceptions (including what “Do Not Resuscitate” actually involves), and discuss how to approach these decisions in a thoughtful, values-based way. The goal is not to tell anyone what choices to make, but to help you feel informed, empowered, and prepared to have these conversations with your loved ones and your healthcare team.
